Background of the Study: Lifestyle modifications, such as diet changes, physical activity, and weight management, are cornerstone interventions for the management of type 2 diabetes mellitus (T2DM) and are crucial for achieving glycemic control (Alhassan et al., 2023). Nurses, as key healthcare providers in the management of diabetes, can play a pivotal role in delivering lifestyle modification programs that promote healthy behaviors among diabetic patients (Abdulazeez et al., 2024). In Sokoto State, where diabetes rates are increasing and healthcare resources may be limited, nurse-led programs could be a cost-effective approach to improving glycemic control among patients with T2DM.
Research has shown that nurse-led programs, which include education on healthy eating, physical activity, and weight management, have a significant positive impact on glycemic control and overall quality of life for diabetic patients (Oluwaseun et al., 2023). However, the specific effectiveness of nurse-led lifestyle modification programs in Sokoto State has not been extensively studied. This study aims to assess the impact of such programs on glycemic control among diabetic patients in Sokoto State hospitals.
Statement of the Problem: Although lifestyle modification is an essential part of diabetes management, many diabetic patients in Sokoto State struggle to implement and maintain these changes. Nurse-led programs could provide the necessary support and guidance to facilitate lifestyle changes, but the impact of these programs on glycemic control remains unclear. This study aims to evaluate the effectiveness of nurse-led lifestyle modification programs on improving glycemic control in diabetic patients.

Scope and Limitations of the Study: This study will focus on diabetic patients in Sokoto State hospitals, evaluating the impact of nurse-led lifestyle modification programs on glycemic control. Data will be collected through patient surveys, blood glucose level assessments, and interviews. Limitations include potential biases in patient reporting and challenges in controlling for confounding factors such as medication adherence.
